If you have a medical question please call our office and follow the phone prompts. You will be asked to leave a voice message and a nurse will respond as soon as possible. DO NOT LEAVE URGENT MESSAGES. If you have a medical emergency call 911.
Please understand that there are many changes in insurance coverage occurring. Some changes you may be aware of and some you may not. We will verify your coverage prior to your office visit and will attempt to inform you of any issues. We participate in almost all insurance plans including Medicare Advantage plans. We do not participate in several of the Humana Plans.
If you are unable to keep an appointment, please call as soon as possible so we may offer that appointment to another patient.
At the time of your visit, you will be asked for information to update your records. We need to verify that we have a current address, phone number, and the correct insurance information. Please bring your insurance cards and driver’s license as you will be asked to allow us to scan them into your account record. At Cardiovascular Consultants we have a strict patient confidentiality policy. Your medical record is strictly private and will not be shared with your employer, friends, or relatives without your written permission.
Patients are expected to pay the required co- pay, deductible and co-insurance at the time of their office visit. We will file your insurance for you and you will receive a monthly statement showing the status of your account. All accounts are expected to be paid within 90 days.
